What is a Diagnex Care Plan?
A Diagnex Care Plan is a coordinated care pathway built around a continuing patient need. It can bring together qualified care management, appropriate doctors, services and follow-up without treating every patient as if they need the same package.
Is a Care Plan the same as a service package?
No. A Care Plan starts with the patient's situation and coordinates the relevant next steps. It is not automatically a fixed bundle of services.
Does a Care Manager decide treatment?
No. Care Managers coordinate and observe within professional competence. Diagnosis, prescribing, medication changes and treatment decisions remain with appropriately qualified treating professionals.
Can every Care Plan be provided in all four cities?
Not necessarily. Diagnex has company-wide presence across Mumbai, Navi Mumbai, Kolkata and Howrah, while individual Care Plan and service availability can vary by locality and requirement.
