Understand the requirement
We start with the procedure requested, available prescription or treating-doctor instructions and the patient’s current situation.
Diagnex coordinates selected doctor-advised clinical procedures in the home setting where the procedure, professional role, required supplies, patient suitability and escalation pathway support safe home-based care.
The home setting can reduce unnecessary travel and make continuing care easier for patients and families. The decision to arrange a procedure at home should still follow the treating professional’s advice and the requirements of that specific procedure.
Each enquiry is reviewed against the requested procedure, clinical instructions, patient situation, locality and operational requirements.
We start with the procedure requested, available prescription or treating-doctor instructions and the patient’s current situation.
The procedure must be appropriate for delivery at home and supported by the required professional, supplies and operating pathway.
Diagnex coordinates the appropriate healthcare professional together with the equipment or consumables required for the confirmed procedure.
Where relevant, observations and procedure information can be communicated to the family and treating team, with escalation when further medical review is needed.
Suitability is considered individually because the same procedure may be appropriate at home for one patient and require a different setting for another.
Diagnex coordinates home-based care within the confirmed clinical pathway. Diagnosis, prescribing and changes to treatment remain with appropriately qualified treating professionals. A Care Manager or visiting healthcare professional does not independently change the patient’s prescribed treatment.
Procedure-specific availability is confirmed when the patient requirement is reviewed.
Tell us what has been advised and where the patient is located. Diagnex can review the requirement and confirm the appropriate next step.
