Your discharge summary is the care plan.

Everything we do in a recovery programme comes from the discharge instructions and the treating doctor. Our nurse turns a two-page summary written in a hurry into something that actually happens at home, and closes the loop back to the doctor.

Daily visits for the first 14 days, then alternate days. Discharge medicine reconciliation on day one, a supervisor review, and one doctor consultation included.

Recovery 90

Recovery 30, then two visits a week for a further 60 days, with a monthly doctor review — for surgery, cardiac events and anything with a longer tail.

Then what

You can stop, or step down into a regular Elder Care visit plan with the same nurse. Nothing renews automatically.

Recovering at any age

Surgery, accidents and cardiac events are not age-specific, and neither is this programme. A forty-year-old home after a bypass or an orthopaedic procedure has exactly the same problem: a long medicine list, a wound, and a follow-up that's weeks away.

Call us before the discharge if you can. The first visit is far more useful on day one than on day four.

Can you start on the day of discharge?

Usually yes, if we hear from you before discharge. Call us while you're still in the hospital and we'll plan the first visit around the discharge time.

What happens after the programme ends?

You can stop, or move to a regular Elder Care visit plan with the same nurse. There's no automatic renewal and no lock-in.

Does this include a doctor?

Recovery 30 includes one doctor consultation and Recovery 90 includes monthly reviews, arranged through Diagnex. Your own treating doctor stays in charge throughout.

Is it cheaper than staying in hospital longer?

Substantially, in most cases — but that's a decision for your treating doctor, not for us. We're here for after they've decided it's safe to go home.

What if a complication develops?

The nurse follows the discharge instructions, contacts the doctor, and helps you get back to hospital quickly if that's what's needed. Catching it early is the entire point.
